1. Acceptance of Terms

By accessing this website, scheduling an appointment, submitting online forms, or receiving services from Kary Dental (“Practice,” “we,” “us,” or “our”), you agree to these Terms & Conditions.

  • Information online does not replace a clinical examination.

3. Establishment of Dentist-Patient Relationship

A dentist-patient relationship with Kary Dental is established only after:

  • An in-person consultation and examination
  • Review of diagnostic imaging (if required)
  • Completion of health history forms
  • Signed treatment consent

     

Submitting a website form, email inquiry, or phone message does not automatically create a provider relationship.

4. Appointments & Cancellation Policy

Scheduling: Appointments may be scheduled by phone or through our online system.

Cancellation Policy: Appointment times are reserved exclusively for you. We require 48 hours’ notice for cancellations or rescheduling. Missed or late-cancelled appointments may result in a cancellation fee. Repeated missed appointments may result in dismissal from the Practice.

5. Financial Responsibility & Payment Terms

Patients are financially responsible for all services provided.

Payment: Payment is due at the time of service unless prior financial arrangements are made.

We accept:

  • Major credit cards
  • Cash
  • Check
  • Third-party financing

     

Insurance: Kary Dental may assist in submitting insurance claims as a courtesy; however:

  • We are not responsible for insurance coverage decisions.
  • Insurance verification is not a guarantee of payment.
  • The patient remains responsible for any unpaid balances.

     

6. Cosmetic Dentistry Disclaimer

Kary Dental offers cosmetic and aesthetic services, including but not limited to:

  • Veneers
  • Smile design
  • Whitening
  • Full-mouth rehabilitation

     

Cosmetic outcomes are subjective and vary by individual anatomy, oral health, and patient compliance. While we strive for excellence, no guarantees are made regarding aesthetic results.

7. Dental Implants & Surgical Procedures

For procedures including implants, extractions, grafting, or other surgical treatments:

  • Risks and benefits will be discussed prior to treatment.
  • Written informed consent is required.
  • Healing outcomes vary based on health, bone quality, and patient compliance.

     

No guarantee of specific results is expressed or implied.

8. Sedation Dentistry

If sedation services are provided:

  • Patients must disclose any additional medical information relevant to sedation, including current medications, prior anesthesia complications, and airway concerns.
  • A responsible adult is required to accompany and transport the patient.
  • Pre- and post-operative instructions must be followed strictly.
  • Failure to follow sedation instructions may result in cancellation or postponement of treatment.

     

9. Membership Plans

If enrolled in a membership or in-house dental plan:

  • Benefits are non-transferable.
  • Membership fees are non-refundable.
  • Plans do not constitute dental insurance.

     

Specific membership terms are outlined in a separate agreement.

10. HIPAA & Privacy Practices

Kary Dental complies with the Health Insurance Portability and Accountability Act (HIPAA) and Maryland privacy laws.

Patient information is protected and handled according to our Notice of Privacy Practices, available in-office and upon request.

Electronic communications (email, online forms, text messaging) may not be fully secure. By communicating electronically, you acknowledge and accept this risk.

11. SMS & Electronic Communication Consent

By providing your phone number or email, you consent to receive:

  • Appointment reminders
  • Treatment follow-ups
  • Billing notifications
  • Practice announcements

     

Message and data rates may apply. You may opt out of text messages at any time by replying STOP.

Consent to electronic communication is not a condition of receiving treatment.

12. Website Intellectual Property

These Terms & Conditions shall be governed by the laws of the State of Maryland.

Any disputes shall be resolved in the appropriate courts located in Montgomery County, Maryland.
